In January 1991, during the Battle of Khafji, U.S. Marine forward observers called for air strikes on Iraqi positions near the Saudi border. Poor radio clarity and shifting coordinates led aircraft to hit friendly vehicles supporting the fight, causing multiple American and allied casualties.

Dust storms and nighttime operations made tracking units difficult. Some crews said they struggled to match map markers with real terrain. The battle was still a coalition success, but the friendly fire incidents pushed commanders to tighten communication rules for future operations.
